Job Description:

We are looking for a compassionate, professional, and motivated Social Care Worker to support individuals who require assistance due to age, disability, illness, or social challenges. The successful candidate will promote dignity, independence, and well-being while delivering personalized care and support services.

Key Responsibilities:

  • Provide person-centered care and support to clients based on individual needs and care plans.
  • Assist with daily activities such as personal hygiene, dressing, eating, mobility, and medication reminders.
  • Encourage and support clients to maintain independence and participate in community and social activities.
  • Develop and maintain trusting relationships with clients, families, and other professionals.
  • Maintain accurate and timely records, including care notes, incident reports, and assessments.
  • Advocate for clients’ rights and needs, ensuring safeguarding practices are followed.
  • Work collaboratively with colleagues, healthcare providers, and external agencies.
  • Respond to emergencies or crises in a calm, responsible manner.
  • Participate in regular training and supervision to ensure best practices are upheld.

Requirements:

  • Relevant qualification in social care, health and social care, or a related field.
  • Previous experience in a care or support role is highly desirable.
  • Understanding of safeguarding, health and safety, and data protection practices.
  • Basic knowledge of mental health, disability, or elderly care issues.
  • Valid driver’s license (if travel is required).
